About this service
The Frailty SDEC provides rapid assessment, diagnosis and treatment without admission to hospital, sending people home on the same day wherever safe, with wraparound support from our community teams where needed.
- SDEC is the provision of same day care for emergency patients who would otherwise be admitted to an acute hospital.
- A Community frailty care model sees patients clinically triaged to require an urgent intervention (that cannot be delivered at home) present to the community SDEC so that relevant conditions can be rapidly assessed, diagnosed and treated without being admitted to an acute hospital, and if clinically safe to do so, go home the same day their care is provided with relevant community support in place.
- The provision of enhanced diagnostic imaging in the community diagnostic and the advances in point of care testing offer significantly increased opportunities to continue to build on the conditions that can be managed.
Referrals
The Frailty SDEC is open for direct referrals from GPs, community practitioners, NHS 111 and SWAST paramedics.
The team will be accepting patients from Redruth and west Cornwall, using the following criteria:
Inclusion
- Frailty score of 5 and above of the ‘Rockwood Clinical Frailty Score’
- Realistic potential to meet same day discharge
Exclusion
- Requires onward escalation of care
- Clinical presentation outside of WCH admission/ transfer risk assessment tool
The service is open from 11am – 7pm, Monday to Friday (including public holidays). The service will accept telephone referrals from 8am.
